Position Summary: Responsible for performing clerical duties and support in accordance with Care Management Services guidelines and procedures.
Basic Qualifications:
Education: High School Diploma or GED equivalent.
Experience:
Prefer some office experience.
Licensure, Registrations & Certifications:

Essential Job Responsibilities:
Perform other related duties as assigned.
Sends, receives, and distribute faxes and emails as appropriate including referral packets and communication with service provider and members.
Assists clients in utilizing community services, including scheduling appointments and assisting with completion of applications for programs for which they may be eligible.
Prints, distributes and revises forms or other documents as directed.
Maintain prompt, accurate and secure documentation as it relates to members’ needs, contacts and plans.
May act as a liaison between the Case Manager, service providers, members and family caregiver as directed by supervisor.
Coordinate or follow up on PCP visits, transportation or request for medical records.
Attend staff meetings, in-services, seminars and workshops as directed by supervisor.
Courteously and professionally answers and redirects all incoming calls as needed.
Performs daily upkeep of office and equipment.
Maintain filing system in an efficient and organized manner.
Communicate with a wide range of constituencies in a diverse community.
