We are seeking a detail-oriented and compassionate Department Assistant to join our Care Coordination team. The ideal candidate will assist in the completion of assessments, support care coordination services, and manage various administrative tasks that keep the department running smoothly.
Job Description:
As an Assistant in the Care Coordination Department, you will play a crucial role in supporting the department’s daily operations. You will assist care coordinators with the completion of assessments, help organize care coordination services and ensure proper documentation and administrative processes are maintained.
Key Responsibilities:
Comprehensive Assessments: Assist in gathering and reviewing information needed for assessments, ensuring that all necessary documentation is collected and organized.
Care Coordination Services Support: Provide administrative support for care coordinators by scheduling appointments, arranging resources, and communicating with clients and service providers.
Documentation Management: Maintain accurate records of client assessments, care plans, and other documentation, ensuring compliance with organizational and regulatory standards.
Administrative Support: Perform general administrative duties including filing, data entry, answering phones, and assisting with correspondence.
Team Collaboration: Work closely with the Care Coordination team to ensure smooth operation of department tasks and client care services.
Administrative Support – Office Culture & Supplies
Qualifications:
High school diploma or equivalent; some college coursework in healthcare or a related field preferred.
Previous administrative experience, preferably in a healthcare setting.
Strong organizational skills with the ability to manage multiple tasks and prioritize effectively.
Excellent written and verbal communication skills.
English/Spanish preferred
