About the Company:
Central Florida Communities, Inc. (CFC) has warm, welcoming, home-inspired facilities and programs for individuals with developmental disabilities in Orange, Brevard, Lake and Seminole Counties.
CFC operates three 64-bed Intermediate Care Facilities for the Developmentally Disabled (ICF/DD’s), Howell Branch Court, Lake View Court and Washington Square. As a member of our staff, you will be a part of one of these living centers and provide vital around-the-clock care, including nursing and medical oversight, to our residents.

Purpose:
Oversee and coordinate Annual Support Plan meetings and ensure responsibility for total implementation of residents’ Support Plan.
Input and update resident data on Therap in conjunction with the Administrative Assistant.
Inform Program Director of any difficulties noted with individual disciplines in the implementation of each resident’s support plan.
Participate in internal auditing and monitor facility compliance with all regulations.
Function as a liaison to parents, day programs, schools, and government agencies.
Supervise the Direct Support Professional Supervisors.
Required Qualifications:
Bachelor’s Degree in Human Services with two years related experience.
Must demonstrate the ability to lift unassisted at least fifty (50) lbs.
