What you’ll do
You’ll be the person who keeps everything running smoothly behind the scenes.
• Match caregivers to patients based on needs and availability
• Handle high-volume calls with professionalism and speed
• Keep patients, families, and caregivers informed and aligned
• Manage call-offs, schedule gaps, and last-minute changes
• Maintain accurate records in HHA Exchange
• Track updates through the Call Dashboard + support pre-billing
• Process referrals and service inquiries
• Ensure care aligns with each patient’s Plan of Care
• Support, guide, and coordinate with care staff
• Flag issues and changes quickly to leadership
What makes you a strong fit
• Experience in scheduling, coordination, or healthcare admin
• Strong attention to detail, you don’t let things slip
• Clear communicator (written + verbal)
• Comfortable juggling multiple priorities at once
• Calm under pressure in high-call environments
• Skilled in Microsoft Office + scheduling systems
• Experience with HHA Exchange is a plus
• Spanish fluency is a plus
