Medical Records Coordinator (General Practitioner)

Position Overview

We are seeking a detail-oriented and proactive Medical Records Coordinator (General Practitioner) to oversee the accuracy, completeness, and compliance of medical records while coordinating daily documentation activities among healthcare professionals. This role is essential in ensuring high standards of patient care, regulatory compliance, and operational efficiency within the clinic.

Key Responsibilities

Medical Records Management

  • Review, monitor, and ensure the completeness, accuracy, and timeliness of all patient medical records on a daily basis.
  • Verify that physicians, nurses, physiotherapists, and other healthcare providers complete documentation according to clinic policies and applicable regulations.
  • Identify missing, incomplete, or inconsistent documentation and coordinate prompt correction with the responsible healthcare provider.
  • Ensure proper coding, documentation standards, and record retention procedures are followed.
  • Conduct regular audits of medical records and prepare reports on documentation quality and compliance.

Clinical Coordination

  • Coordinate with doctors, nurses, physiotherapists, nutritionists, and other healthcare professionals to maintain high-quality clinical documentation.
  • Act as the primary liaison between clinical departments regarding medical record requirements and documentation standards.
  • Monitor daily patient documentation workflows to ensure records are updated in a timely manner.
  • Assist in implementing and improving documentation processes, forms, and electronic medical record (EMR) systems.

Quality Assurance & Compliance

  • Ensure compliance with healthcare regulations, accreditation standards, and clinic policies regarding medical records.
  • Support accreditation preparation and quality improvement initiatives related to medical documentation.
  • Develop and update medical record policies, procedures, and documentation guidelines.
  • Provide training and guidance to healthcare staff on proper documentation practices.

Reporting & Administration

  • Prepare daily, weekly, and monthly reports on medical record completion rates, compliance metrics, and documentation quality.
  • Track outstanding documentation and follow up with responsible healthcare providers.
  • Participate in clinical quality meetings and contribute recommendations for continuous improvement.

Qualifications

  • Medical Doctor (MD) / General Practitioner with a valid medical license (STR).
  • Minimum 1–2 years of clinical experience preferred.
  • Strong understanding of medical documentation standards, healthcare regulations, and clinical workflows.
  • Experience with Electronic Medical Records (EMR) systems is preferred.
  • Excellent organizational, analytical, and communication skills.
  • Strong attention to detail and ability to work collaboratively with multidisciplinary healthcare teams.
  • Proficient in Microsoft Office and healthcare information systems.

Preferred Qualifications

  • Experience in medical rehabilitation, physiotherapy, or outpatient clinic settings.
  • Experience with healthcare accreditation standards and quality management systems.
  • Familiarity with BPJS documentation requirements and insurance-related medical records.

  • Daily medical record completion rate.
  • Documentation accuracy and compliance score.
  • Timeliness of medical record closure.
  • Audit findings and corrective action completion rate.
  • Accreditation and regulatory compliance performance.
